AJCC 9 (bladder) pN summary: - pN0: No regional lymph node metastasis. - pN1: Metastasis in a single regional lymph node. - pN2: Metastasis in multiple regional lymph nodes. - pN3: Metastasis in common iliac lymph node(s).
Note: Report number of positive nodes, size of deposits, extranodal extension. Lymph node status upstages to stage III/IV depending on T and M.
Any pM1 = Stage IV. Document site(s), number, and method of confirmation (biopsy/imaging).
AJCC 9 Stage Group Guidance (expanded)
Simplified AJCC 9 anatomic stage grouping for bladder: - Stage 0: pTa or pTis, N0, M0. - Stage I: pT1, N0, M0. - Stage II: pT2, N0, M0 (muscle-invasive). - Stage III: pT3 or pT4a or any N1–N2, M0 (perivesical extension, invasion of prostatic stroma or adjacent organs limited). - Stage IV: pT4b (fixed invasive to pelvic wall/abdomen) or any M1.
Note: For TURBT specimens, muscle sampling is often incomplete; recommend clear statement about presence/absence of muscularis propria and consideration of re-resection when muscle absent or T1 with high-grade disease.
