AJCC 9 pT (colon) — pTis = mucosa only; pT1 = invades submucosa; pT2 = invades muscularis propria; pT3 = penetrates through muscularis propria into subserosa or pericolorectal tissues; pT4a = tumor penetrates to surface of visceral peritoneum; pT4b = tumor directly invades other organs/structures. Use yp prefix for post‑treatment. Document serosal involvement, direct organ invasion, and measurement methods.
Lymph Nodes
AJCC 9 pN (colon) — pN0 = no regional node metastasis; pN0(i+) = ITCs only (<0.2 mm); pN1mi = micrometastasis (0.2–2.0 mm); pN1 = metastasis in 1–3 regional nodes OR tumor deposits N1c when nodes negative; pN2 = metastasis in ≥4 regional nodes (pN2a/b subdivisions by node count in AJCC 9). Record node counts, micrometastasis, ITCs, tumor deposits (N1c), and ENE.
Biomarkers & Molecular
Note: MMR/MSI status affects prognosis and treatment (immunotherapy eligibility). Record method (IHC vs PCR/NGS) when available.

Stage IV: any T, any N, M1 (AJCC 9 subcategories M1a/b/c apply for distribution of distant disease).
