Lung Cancer Synoptic Report — CAP elements with AJCC 9 staging (automatic)

Case / Specimen
Tumor / Pathologic Findings
AJCC 9 (lung) pT summary: Tis = carcinoma in situ; T1 subdivided by size (T1a ≤10 mm, T1b 11–20 mm, T1c 21–30 mm); T2 subdivided (T2a 31–40 mm, T2b 41–50 mm) and includes invasion of visceral pleura or main bronchus ≥2 cm distal to carina; T3 = 51–70 mm or separate tumor nodules in same lobe or chest wall invasion; T4 = >70 mm or invasion of mediastinum, heart, great vessels, trachea, esophagus, vertebral body, diaphragm, or separate tumor nodules in different ipsilateral lobe. Use yp prefix for post‑treatment. For precise definitions and qualifiers consult AJCC 9.
Regional Nodes
AJCC 9 (lung) nodal summary: N0 = no regional nodal metastasis; N1 = metastasis in ipsilateral peribronchial and/or ipsilateral hilar and intrapulmonary nodes (including involvement by direct extension); N2 = metastasis in ipsilateral mediastinal and/or subcarinal nodes; N3 = metastasis in contralateral mediastinal, contralateral hilar, ipsilateral or contralateral supraclavicular nodes. Record stations, counts, and ENE. N category influences stage grouping per AJCC 9 tables.
Distant Metastasis
AJCC 9 (lung) pM: M0 = no distant metastasis; M1a = separate tumor nodule(s) in contralateral lung, tumor with pleural/pericardial nodules or malignant effusion; M1b = single extrathoracic metastasis in single organ; M1c = multiple extrathoracic metastases in one or more organs. Any M1 = Stage IV (subclassified by M1a/b/c).
AJCC 9 Stage Group Guidance (lung) — text included
Representative AJCC 9 stage grouping (simplified, used by auto-calculator): - Stage 0: pTis, N0, M0.
- Stage I: T1–T2a, N0, M0 (IA/IB subdivided by T size cutpoints).
- Stage II: T2b–T3, N0, M0 or T1–T2 with N1 (IIA/IIB by exact combos).
- Stage III: T3–T4 and/or N2–N3, M0 (substages IIIA/IIIB/IIIC determined by exact T and N combos).
- Stage IV: Any T, any N, M1 (subgroups by M1a/b/c). Note: Lung AJCC staging uses size cutpoints and nodal station locations for exact substage assignments per AJCC 9 tables — this tool approximates stage group for rapid reporting. Always confirm with AJCC 9 tables.
