Primary tumor / Ovarian findings
AJCC 9 (ovary/fallopian tube/peritoneum) pT/FIGO summary (expanded): - pT1: Tumor limited to ovaries (one or both) and fallopian tubes (confined to pelvis). Subclassify: - pT1a: Tumor limited to one ovary, capsule intact, no tumor on surface, negative washings. - pT1b: Tumor limited to both ovaries, capsules intact, no tumor on surfaces. - pT1c: Tumor limited to one or both ovaries with surgical spill, capsule rupture, tumor on ovarian surface, or positive peritoneal washings. - pT2: Tumor involves one or both ovaries with pelvic extension (below pelvic brim) or primary peritoneal disease confined to pelvis. - pT3: Tumor involves one or both ovaries with microscopically or grossly confirmed peritoneal metastasis beyond the pelvis and/or regional lymph node metastasis. Subclassify by size/extent of peritoneal implants. - pT4 (when used by AJCC): Tumor with distant metastasis (extra‑peritoneal) — many classifications use M categories for distant disease.
Note: For ovarian tumors, record presence/size/location of peritoneal implants, positive peritoneal washings/ascites, and lymph node results. Use FIGO/AJCC tables for precise subclassification.
Peritoneal / omental disease
Expanded notes for peritoneal disease and pM: - Peritoneal metastasis beyond pelvis typically maps to higher T (pT3) and influences stage grouping. - Positive peritoneal cytology/washings historically affected staging; record results (some staging systems no longer upstage for positive cytology alone, verify AJCC 9 guidance). - Omental involvement and macroscopic peritoneal implants should be documented with size and sites — these determine T3 subclassification and overall stage. - Distant extra‑peritoneal metastases (e.g., pleural) are M1 (Stage IV).
Regional lymph nodes
AJCC 9 pN summary (ovarian): - pN0: No regional lymph node metastasis. - pN1: Regional lymph node metastasis present (pelvic or para‑aortic nodes). Record number of positive nodes, stations, size of nodal deposits, and extranodal extension.
Nodal involvement commonly increases stage to stage III when peritoneal disease also present; follow AJCC tables for exact grouping.
Distant metastasis
AJCC 9 pM summary: - pM0: No distant metastasis identified. - pM1: Distant metastasis present (extra‑peritoneal sites such as pleura, lung, liver).
Any M1 = Stage IV. Document site(s), number of lesions, and method of confirmation (biopsy/imaging).
Note: Positive pleural fluid cytology or diaphragmatic involvement should be documented and interpreted per AJCC guidance.
AJCC 9 Stage Group Guidance (expanded)
Simplified AJCC 9 anatomic stage grouping for ovarian/fallopian tube/primary peritoneal carcinoma (guidance — confirm with official tables): - Stage I: Tumor limited to ovaries/fallopian tubes (pT1), N0, M0. - Stage II: Tumor involves one or both ovaries with pelvic extension (pT2), N0, M0. - Stage III: Tumor involves one or both ovaries with peritoneal implants beyond the pelvis and/or regional lymph node metastasis (pT3 and/or pN1), M0. - Stage IV: Distant metastasis (pM1), including pleural effusion with positive cytology or parenchymal metastases.
Use AJCC 9 / FIGO tables for precise substaging, particularly for pT1 subcategories and pT3 subclassification by implant size/extent.
