Pancreatic Exocrine Carcinoma Synoptic Report — CAP elements with AJCC 9th Edition (auto stage)

Tumor / Pathologic features
AJCC 9 (pancreas) pT summary (expanded — site-specific definitions apply): 
- pT1: Tumor ≤2 cm in greatest dimension and limited to pancreas. 
- pT2: Tumor >2–4 cm and limited to pancreas. 
- pT3: Tumor >4 cm OR tumor of any size extending beyond pancreas (peripancreatic soft tissue) without involvement of celiac axis or SMA. 
- pT4: Tumor involves the celiac axis, common hepatic artery, or superior mesenteric artery (unresectable arterial involvement). 

Notes: - Venous involvement (SMV/portal vein) may be present in resectable or borderline‑resectable disease and does not by itself define pT4; document level and whether tumor invades/encases vessel wall. - For neoadjuvant specimens use yp prefix and document treatment effect.

Regional lymph nodes
AJCC 9 (pancreas) pN summary: 
- pN0: No regional nodal metastasis. 
- pN1: Regional lymph node metastasis present (record number positive and total examined). 

Notes: 
- Size of nodal deposits and extranodal extension should be reported. 
- Increasing nodal burden correlates with worse prognosis; use AJCC tables to combine T and N for staging.
Distant metastasis
AJCC 9 (pancreas) pM summary: 
- pM0: No distant metastasis identified. 
- pM1: Distant metastasis present (biopsy-proven when possible). 
     Subclassification sometimes used by site (e.g., pM1a/b for limited vs multiple sites) — any pM1 = Stage IV. 

Common metastatic sites: liver, peritoneum, lung. Document site(s), number of lesions, and method of confirmation (biopsy/imaging).
AJCC 9 Stage Group Guidance (expanded)
Simplified AJCC 9 anatomic stage grouping for pancreatic exocrine carcinoma (guidance — verify with AJCC 9 tables): 
- Stage I: pT1–pT2, N0, M0 (I subdivided by size). 
- Stage II: pT3, N0 or pT1–T3 with N1, M0 (site-specific combinations apply). 
- Stage III: pT4 and/or more extensive regional disease (site-dependent). 
- Stage IV: Any M1. 

Use the AJCC 9 staging tables to combine exact T and N categories and for neoadjuvant (yp) staging adjustments.
