Primary tumor / Prostate findings
AJCC 9 (prostate) pT summary (expanded): 
- pT0: No evidence of primary tumor in surgical specimen. 
- pT2: Organ-confined disease (involves prostate only). 

Subcategories are no longer subdivided by lobes in AJCC 8/9 commonly used in practice; record organ-confined with tumor confined to prostate. 

- pT3: Extraprostatic extension beyond prostatic capsule. 
- pT3a: Extraprostatic extension (EPE), unilateral or bilateral, or microscopic invasion of periprostatic tissue, or microscopic invasion of bladder neck mucosa. 
- pT3b: Seminal vesicle invasion (SVI). 
- pT4: Tumor invades adjacent structures other than seminal vesicles (e.g., external sphincter, rectum, levator muscles, pelvic wall). 

Note: Document extent and laterality of EPE, distance from margin, focal versus extensive SVI, and margin site(s).
Regional lymph nodes
AJCC 9 (prostate) pN summary: 
- pN0: No regional lymph node metastasis. 
- pN1: Metastasis in regional lymph node(s). 

Record number of positive nodes, total nodes examined, size of nodal metastases, and extranodal extension. Regional nodal disease upstages to at least Stage IV when distant metastasis absent? (See AJCC tables for combining T and N). Note: Distinguish limited nodal micrometastasis from macrometastasis; record nodal station(s) involved.
Distant metastasis
AJCC 9 (prostate) pM summary: 
- pM0: No distant metastasis identified. 
- pM1: Distant metastasis present. 

Subclassification may include pM1a (non-regional lymph node), pM1b (bone), pM1c (other sites such as lung, liver). Any pM1 = Stage IV. Document site(s), number of lesions, and method of confirmation.
AJCC 9 Stage Group Guidance (expanded)
Simplified AJCC 9 stage grouping for prostate cancer (guidance — confirm with official tables and prognostic staging which incorporates grade/PSA): 

- Stage I–II: Organ-confined disease (pT2) with lower grade and PSA (prognostic grouping required for final stage). 
- Stage III: pT3 (EPE or SVI), N0, M0. 
- Stage IV: pT4 and/or N1 and/or M1. 

Note: For prostate cancer, final prognostic stage group incorporates grade group and PSA; this tool provides anatomic TNM stage only.
